HT Family Physicians treats patients of all ages and is pleased to provide families in our community with comprehensive, quality medical care in a comforting environment.

Our office uses an electronic health record system that allows for an enhanced patient experience.

We assist patients with managing chronic medical concerns such as diabetes, hypertension, depression, and cholesterol. In addition, our physicians on staff believe in the importance of preventive care as a means to maintaining good health and quality of life.

Your First Appointment

On your first visit, we will begin a permanent record. Please bring any medications you are taking, so that we may accurately determine the dosage and frequency. We will ask you to complete a medical history and registration form, which you can find on this website and fill out ahead of time. Please bring your insurance card and try to arrive at least fifteen minutes early for your first visit.

Late to Appointment Policy

We value your time and strive to see you as close to your appointment time as possible. Recognizing unanticipated things happen, we will hold your appointment for a 10 minute grace period after the scheduled start time of your appointment. In consideration of every patient, all appointments will be automatically canceled thereafter.

If you arrive after your appointment has been canceled, you will be asked to reschedule the appointment. If you are willing to wait and a provider can accommodate it, we will try to provide you with a partial or full appointment. Priority is given to our scheduled patients, which means you may have to wait. If this is not convenient for you, you may choose to reschedule.

Canceling an Appointment

If you are unable to keep your appointment or you are going to be late, please call our office as soon as possible. This courtesy allows us to be of service to other patients.

Missed Appointment or “No-Show” Policy

It is your responsibility to remember your scheduled appointment. After three (3) missed appointments, we may choose to discontinue your care.

Prescription Refills

Our preferred method for refilling prescriptions is to have your pharmacist request a refill on your behalf. We are unable to refill routine prescriptions after normal business hours.

After Hours Care

In an Emergency or life-threatening situation call 911. Otherwise, if you need medical care after hours, during holidays or weekends call our office BEFORE going to an Urgent Care. Our Physician on-call will treat or direct you to the most appropriate care. We might be able to save you time and money.

Confidentiality

At HT Family Physicians we have a strict patient confidentiality policy. Your medical record is strictly private and will not be shared with your employer, friends, or relatives without your written permission.
